Perhaps the earliest memories I have are of being a stubborn, determined child. Through the years my mother has told me that it was fortunate that I chose to do acceptable things, for if I had chosen otherwise, no one could have deflected me from my path.

We still live in a world in which a significant fraction of people, including women, believe that a woman belongs and wants to belong exclusively in the home; that a woman should not aspire to achieve more than her male counterparts and, particularly, not more than her husband.

Patients who are being kept alive by technology and want to end their lives already have a recognized constitutional right to stop any and all medical interventions, from respirators to antibiotics. They do not need physician-assisted suicide or euthanasia.

A health system that lacks commodities for managing high-mortality infectious diseases and the main killers of mothers and young children will not have an adequate impact. By the same token, even the best-stocked delivery system will have an inadequate impact if it fails to reach the poor.
